Ontario Licensed Real Estate Agents and Maternity Leave


Most Ontario real estate agents work solely on a commissions basis. These agents are generally on an independent contractor status making them self employed. The "employed" rules and regulations in the province of Ontario will for the most part, not apply to self employed real estate sales people who decide on taking a maternity leave.

When a real estate agent, Realtor, sales representative, or Broker, considers her maternity leave situation, she will be thinking about her loss of income, existing and continuing monthly expenses, and possibly the option of not returning to realty sales for a longer period of time.

Here is a typical scenario for an Ontario Realtor who is on maternity leave:

Most likely she was a registered real estate agent in Ontario with a franchise Brokerage. That Brokerage is a member of the Toronto real estate board or depending on their location in Ontario, their own local board. As a board member the Brokerage pays its dues including associations like O.R.E.A and C.R.E.A.. Therefore, all agents in that Brokerage will also be responsible for those dues whether active in sales or not. The agent on Maternity leave, then, will also be paying these dues so long as she remains registered with this Brokerage.

Not all franchise Brokerages are equal, but most of them will have certain common expenses or fees that their registered sales agents must pay as part of their agreement. These fees could be desk fees, advertising fees, franchise and transaction fees, fixed monthly office fees or administration costs etc., etc. Although the real estate boards fee and dues are mandatory whether you are on maternity leave or just not active in sales, some Brokerages may allow non payment or waive some of the listed expenses while the agent is on maternity leave.

An agent on maternity leave may have some legitimate concerns about having to pay for the same expenses she did while in the sales field. Another concern may be the loss of income due to her leave. Still another thought may be the eventual feeling, while taking care of her baby and family, that returning to real estate sales will not be something she wants to do sooner but rather, much later on.

There is a lot of flexibility in Ontario with real estate agents ready to take maternity leave. If expenses are going to be a problem, the licensed agent can opt to resign from her current Brokerage and register with a non board member realty Brokerage. She can park her license with a Brokerage that will not have any of the above mentioned fees and dues applicable. This alone will save the agent a lot of money especially if her maternity leave is extended for an uncertain period of time.

Income while on maternity leave is possible whether the realty agent remains registered with a board member Brokerage or non member Brokerage. She can refer business to a fellow real estate agent or Brokerage and agree to accept a referral fee. As long as her real estate license is registered with a Brokerage in good standing with R.E.C.O., she can continue to earn referral commissions paid to her through her Brokerage.

Although maternity leave is not an option for many agents, it's good to know that in Ontario, real estate agents who elect to take maternity leave can do so with a little piece of mind. The reassurance that these agents can substantially reduce their expenses and still earn an income by referring sales to other licensed agents, surely becomes important considerations as they decide to become stay at home parents.

Future of Depression Treatment - What Lies Ahead?


Today I'm looking into the future of depression, and so far it doesn't look that good.

Depression will become a second major cause of all deaths after a heart disease by 2020, according to the World Health Organization (WHO) prognosis. These days depression affects all ages -toddlers, teenagers, young generation, middle-aged people and our elders.

The symptoms of depression, I truly believe, weaken the whole body, and it becomes a vicious circle: the more you're depressed the more you're ill, the more you're ill - the more you're depressed. In fact, there is some research looking into the connection between the inflammatory disorders (for example, rheumatoid arthritis, hay fever, atherosclerosis) and depression, especially bipolar disorder. The thought is to try and see if immunotherapy will help in both cases.

So, what lies ahead for the future of depression treatment?

- The scientists are researching the gene behind the depression. The idea is simple - to try new medications first and adjust the dose on this gene, instead of going through months of different antidepressants in hope that one of them will work for the patient.

- ECT (Electroconvulsive Therapy or shock therapy), which is given to the patients with severe depression. What happens is that you are anesthetised, then electric currents released for 40 seconds into your brain, which causes mini seizures, which in turn help releasing a lot of neurotransmitters into your brain (among those serotonin, dopamine and noradrenaline that improve our mood). Of course, there are side effects as well, and one of those like with all seizures is a short memory loss. The researchers now experiment with narrowing the electric waveform to improve the results.

- TMS (Transcranial Magnetic Stimulation) works a bit like ECT stimulating the release of neurotransmitters into the brain with the help of magnetic impulses similar to those in MRI scan. It still mostly in experimental stage, and I heard that the treatment costs about $ 6,000(!) requiring daily sessions for 4-6 weeks. There are mostly no side effects apart from sense of discomfort and occasional headache. Let's hope with more research the treatment will become more affordable.

- Obviously, the antidepressant drugs are here for a long run as well. There will be more new pills, hopefully with less side effects.

Please do not over-exercise as it can disturb the body's hormonal balance.

c) Patience

Research had indicated that women who are anxious about getting pregnant conceive less often than women who are patient. The ability to get pregnant has a lot to do with hormones and stress levels. Anxiety can impact both of these adversely.

The Most Frequently Asked Questions About Breastfeeding


As an OB, my patients ask me just as? many questions about breastfeeding as they ask about pregnancy itself. I don't mind at all because it is such an important topic. Over the years, I have scribbled many of these questions in my notebook so that I could research the most current information for new mothers eager to learn how to feed their babies the natural way.

Now, these questions are the basis for this week's?article which is written just for you, with a completely different mindset and format than all my previous articles. Before writing however, I decided to interview the head lactation consultant, Ms. Renate Abstoss at Stamford Hospital in Connecticut, to get a second opinion and gather her responses. Ms. Abstoss sat for the first International Board Exam for Lactation Consultants in 1985 and has continuously worked in the field since that time in California, New York, Germany, Austria, Switzerland, and Connecticut. She was the first lactation consultant appointed as State Certified Teacher for Bavarian Midwifery schools.

There are probably many variations of responses to these questions so please consider this information helpful, but not absolute. I welcome any additional "pearls" from other lactation consultants or experienced moms so we can share the knowledge and promote the best nutritional health for babies everywhere.

What are the most common problems that women encounter when pumping?

Dwindling milk supply is probably the foremost problem that women encounter when pumping milk. This can be attributed to many different factors.

Infrequent pumping

A working woman should try and pump every 3 hours for 10 to 15 minutes or at least for every missed feeding

Inefficient pumping

Pump being used may have inadequate pressure or not enough cycles per minute causing a decrease in the milk supply.

Lack of breast feeding

Even when mom is home from work, she should try and breast feed the baby to stimulate milk production. For convenience, babies may be bottle fed too frequently.

Painful nipples

This can be due to a bad pump due to excessive suction pressure, pumping for too long a period of time, or poor latch when the baby does breast feed.

Fatigue and exhaustion

The stress of taking care of a newborn along with pressure from work and home can result in exhaustion and fatigue which can decrease the milk supply.

How do I increase my milk supply?

There are several medications and herbal products available to help with increasing the milk supply. To increase an existing milk supply, one can take two herbal supplements in combination fenugreek and blessed thistle, both of which are available in health food stores and have no known contraindications.

The two most commonly used prescription drugs are Metoclopramide and Domperidone both of which were designed as stomach medications but were found to increase prolactin production in the brain. The medication Domperidone, (Motlium) is not approved for use in the United States and the FDA has issued warnings against the use of this drug as a galactagogue because in higher dosages, when given intravenously, Domperidooe it was associated with cardiac arrhythmias and cardiac arrest. However, the small dosages that are used when given orally to increase lactation (30-40 mg/day) and for the short duration of three to six weeks does not seem to be a great concern. Patients generally obtain this medication from Canadian pharmacies or from Mexico and Europe where it is over the counter. Studies are now being conducted by ILCA (International Lactation Consultant Association) regarding the safety of domperidone.

Metoclopramide (commonly known as Reglan), used to treat severe nausea in pregnancy, has been known to have a side effect of depression so its use has to be carefully monitored in the postpartum mother. The drug can be detected in breast milk and the long term side effects to infants is unknown.

How do I treat plugged milk ducts?

Milk ducts that do not drain can cause the milk to back up resulting in a plugged milk duct. Often the the surrounding breast tissue becomes hardened and inflamed. This area can eventually become infected resulting in a mastitis requiring antibiotic therapy. Plugged ducts often occur when:

Mom misses feedings

Mom fails to pump frequently

The breast fails to empty

Nursing more frequently

Changing positioning

Improving the latch of the baby (sometimes a shield is necessary). A lactation consultant would be very instructive in this situation.

This can be a result of incorrect positioning, inadequate pressure from a pump, or restriction of milk flow from poorly fitting bras or any kind of breast trauma that damages the duct. Fatigue, stress, or failure of the baby to latch on properly can also contribute to this problem.

Standard Treatment consists of moist heat beforehand for approximately 5 minutes, empty breast as much as possible, cool compresses afterwards, and reduce swelling in between feedings. Massaging the duct towards the nipple may also be helpful. If the plug comes out, it may look like dried milk in a string. One should continue to nurse to fully clear the plug. Apply cold compresses via an ice pack or a plastic bag of frozen peas for approximately 15 to 20 minutes. Fresh cabbage leaves to the area is another herbal remedy that can be used in between feedings to help with engorgement. Cabbage leaves can be left in the bra for approximately one hour until it wilts. It is important to avoid the nipple area when applying the cabbage leaf because the cabbage is caustic and can cause irritation

What do I do if the baby does not latch on?

Latch problems can depend upon the situation causing them:

It may be due to a newborn who is just learning how to latch.

Assistance from a lactation consultant can be very helpful in making sure the baby is latched deeply with a mouth very wide to get all the breast tissue. The baby should be latched well beyond the tip of the nipple. Often latching problems occur because the baby fails to flare the bottom lip.

Is it due to flat or inverted nipples?

If the problem is due to flat or inverted nipples, a suction device, pump, or nipple shield may be used to avoid engorgement by expressing the milk. This will avoid a lowered or disappearing milk supply. Different lactation consultants and hospitals may prefer one modality over another, however all agree that care has to be taken when using any device that serious trauma or damage does not occur to the nipple.

Once the baby is "educated" that the breast is his or her "food supply", generally the baby will nurse fine. It is helpful to try and get beyond the first few days when only colostrum is present, until the milk comes in. Try and avoid avoid artificial nipples at the very beginning.

Is the baby not hungry at this time?

Is there a nursing strike?

A "nursing strike" can occur if a well nursing baby, beyond the newborn period suddenly refuses to nurse. This can occur often after 4 months of age when a mother will state "my seven month old baby just suddenly weaned herself." These babies are not weaning but are actually facing a situation where nursing has become unpleasant, boring, or painful. Often these infants need to be taught to return to a functional breastfeeding pattern with increased skin to skin contact, trying to nurse in a quiet, calm environment without distractions, such as nursing at night in bed. Bottles should be avoided but spoon or cup feeding with expressed milk is acceptable. If the underlying cause is treated, most babies will happily go back to their normal nursing pattern.

Is the baby getting sick?

Causes of poor latch can be an undiagnosed ear infection, a urinary tract infection, or an upper respiratory infection. Check with your pediatrician to be sure an illness is not being missed. Maternal stress can also cause this problem.

Regardless of the problem, it is important to remove the milk as efficiently as possible if an established supply exists to avoid engorgement which will lower or cause the milk supply to disappear.

If the baby is not latching, it is important to stimulate the establishment of milk production through use of a pump.

How do I wean the baby when I have decided to transfer to a bottle?

Weaning is the time the baby transitions away from the breast. Time frames for weaning can vary from weeks to months.

Slow weaning is always more optimal. Some mothers will use the pump for weaning and stop breastfeeding entirely. One option for weaning would be to reduce the time per pumping, ie instead of pumping for 15 minutes, reduce it to 10 minutes. This can gradually be dropped down to 2 to 3 minutes per session. An alternative way to wean is to increase the interval between pumping sessions, ie from every 3 hours to every 4, 6 or even 8 hours. If desired, one can use ice packs after pumping for comfort. Also, Motrin can be used for anti-inflammatory pain relief. A comfortable, tight bra can also help with support and relief. Nursing pads can be used to prevent excess leakage. A recommended herbal remedy can be drinking 3 cups of sage tea which is available in health food stores. It has a strong and astringent taste so adding honey makes it more palatable. Do not use this if currently pregnant since it has abortive action.

On occasion, sudden weaning becomes necessary. Discomfort is generally greater since the breast continues to make milk and the breast remains engorged. Nursing can be replaced with pumping but only pump the breast to allow comfort and not to completely drain. Increase time frame between pumpings.

Three days of Sudafed can help dry up the milk production. The milk supply decreases as you feed less. The milk quality also changes becoming more salty and colostrum-like which many babies do not like. Consequently, this also helps diminish the nursing.

Sleep During Pregnancy - Advice to Help Pregnant Women to Sleep Better


As a pregnant woman I am sure that you have received more advice than you can even remember. Some good, some bad, but if you are having difficulty getting quality sleep during pregnancy please listen to what I have to say.I promise that if you give me a couple minutes of your time I can help you to catch those elusive ZZZ's during pregnancy.

Let me first say that I do know what I am talking about. I am the mother of two beautiful children but let me tell you one of the biggest challenges for me when I was pregnant was getting a good night's sleep.

It started during my first pregnancy when I was pregnant with my son. Other than having to get up to pee more often I slept fine during the first couple months of my pregnancy. But as the months progressed I found it more and more difficult to sleep.

I just could not find a comfortable position to get into. I am a natural back sleeper but found it too uncomfortable to sleep in that position. I would get up the morning tired, cranky, sore, and frustrated from another uncomfortable night of sleep.

I hit the Internet and started doing some research. I found that sleep during pregnancy is a challenge for a lot of expectant mothers; I felt a little better knowing I was not alone. I learned that doctor's, for many health benefit reasons, recommend women sleep on their left side when pregnant. I also discovered that many expectant mothers recommended maternity pillows to help get a comfortable night of sleep.

Now I had tried using pillows to help me get comfortable to no avail. But as I looked more into maternity pillows I discovered that they were full body pillows specially designed to offer support and comfort to pregnant women. I found more and more positive reviews of maternity pillows and women who swore by them as the best thing they had found.

I consider myself to be fairly frugal but this was one indulgence I was going to allow myself. I needed my sleep and if a maternity pillow was going to help then, by golly, I was going to get one. So I found the highest rated one and placed my order.

What a sweet relief! I went to bed that night with my big comfy maternity pillow and slept like a baby for the first time in weeks. It was just like I had read; it offered me support and comfort tin all the needed places and helped to keep me in the side sleeping position that doctors recommend. My sleeping problems have been solved! I used it for the rest of my pregnancy with my son and pulled it out of the closet and used it when I was pregnant with my daughter.

So, if you are having difficulty with sleep during pregnancy please take my advice and get a maternity pillow. You will not regret it. My pillow was my best friend when I was pregnant. Any expectant mother who cannot sleep well should definitely invest in one of these wonderful pillows.

Lupus and Pregnancy - What to Consider Before You Decide to Have a Baby


If you are living with lupus and contemplating having a baby there are a lot of questions you are going to want to address.

Here are the top concerns I get emailed about, and those that I will answer in my mini-series on lupus and pregnancy. (For more information right away, please visit my site below).

    -Is it safe to have a baby if I have lupus?

    -Will my baby get lupus too?

    -Will getting pregnant cause a lupus flare? When is the best time to get pregnant?

    -Can my baby be born naturally?

    -Can I breast feed?

    -Is there a greater likelihood of miscarriage?

    -What is toxemia of pregnancy?

    -Is there something I should do to help the pregnancy process?

    -I am afraid I will get a flare from being pregnant. What if I can not take care of my child?

This article will address some of the most common questions addressed above. (Stay tuned for part 2 to follow).

1. Is it safe to have a baby if I have lupus?

During the early seventies, women were told if they had lupus they should not get pregnant. Since those days, treatment and support for lupus patients have changed however it is still an important question that must be addressed, looking at your specific illness, needs and risk factors.

Because lupus is a disease that strikes young women, predominantly in their childbearing years, pregnancy is a big concern as you well imagine. However, for most women living with lupus a successful pregnancy is possible. That being said there are 'pregnancy dos and don'ts' that you must follow during your pregnancy to avoid complications.

Here are some suggestions to support yourself before and during pregnancy:



    Carefully plan your pregnancy, taking into consideration all of your needs. (For example, sufficient time off of work, support taking care of your child, sufficient time for rest, daily chores support, a house cleaner to do the most strenuous work, sufficient funds to allow for extra costs such as a nanny for the first few months etc.)

    Ideally, it is best that your lupus be under control or in remission for at least 6 months before you get pregnant.

    Have a strong support system in place you can rely on (For example, during a postpartum period, a stressful period or just coping with life changes etc.).

    Keep a diligent record of your condition, symptoms and health fluctuations.

    Plan your calendar in advance to ensure all of your necessary activities are taken care of well in advance to avoid stress. (For example, preparing the baby's room, collecting the necessary baby items such as car seats, strollers, etc. well in advance to avoid undue stress and rushing).

    Create a stress-free process as much as possible to support and maintain a healthy immune system, including dietary needs. (For example, an organic foods delivery system is a great way to avoid regular grocery trips).

    Take extra care of your health needs, following a nutritious healthy lupus diet, with supportive supplements (such as essential fatty acids to naturally reduce inflammation) and prepare your meals in advance (store extra meals in the freezer) to avoid extra work in the kitchen.

    Avoid environmental toxins (such as painting fumes etc.) and be extra careful with situations that don't support wellness (such as cleaning products, second hand smoke or other noxious items).


Lupus pregnancies are considered high risk!

Being pregnant while living with lupus is considered a high-risk pregnancy. As mentioned above, it is especially important that you find an obstetrician (OB) who is experienced in managing high-risk pregnancies and someone you feel confident in. This person may or may not be your regular OB; however having an expert on hand (at minimum for a second opinion) is highly advisable

The biggest risk to your baby is premature birth. Births before 36 weeks are considered premature.

Here are some statistics concerning the risks of pregnancy and lupus:

About 50% of lupus pregnancies end before 40 weeks (9 months), usually because of the complications previously discussed.Babies born after 30 weeks, or weighing more than 3 pounds, usually do well and grow normally Premature babies may have these problems, but in modern neonatal units they can be easily treated:

-Difficulty breathing

-Developing jaundice

-Becoming anemic

Even babies as small as 1 pound, 4 ounces have survived and have been healthy in every way; but the outcome is uncertain for babies of this size. There is one congenital abnormality that occurs only to babies of lupus mothers (neonatal lupus, described below)There is no unusual frequency of mental retardation in babies of lupus mothers

Of course, the most important question lupus patients as is: will my baby be ok? The answer is, in most cases, yes. Babies born to women living with lupus have no greater chance of birth defects or mental retardation than any other baby.

Approximately 3% of babies born to moms with lupus will also have neonatal lupus. This will include temporary rash and abnormal blood counts. This usually will disappear by the time the child is between 3 and 6 months old and will not recur.About 50% of babies with neonatal lupus are born with a heart condition. This condition is typically permanent and treatable with a pacemaker. You should discuss this possibility with your doctor.About 33% of people with lupus have an antibody known as the anti-Ro, or anti-SSA, antibody.About 10% of women with anti-Ro antibodies-about 3% of all women with lupus-will have a baby with a syndrome known as "neonatal lupus."

Neonatal lupus is not SLE. Neonatal lupus consists of:

-A transient rash

-Transient blood count abnormalities

-Sometimes a special type of heart beat abnormality. If the heart beat abnormality occurs, which is very rare, it is treatable but it is permanent.

Neonatal lupus is the only type of congenital abnormality found in children of mothers with lupus.

For babies with neonatal lupus who do not have the heart problem, there is no trace of the disease by 3-6 months of age, and it does not recur. Even babies with heart beat abnormality problems grow normally. If a mother has had one child with neonatal lupus, there is about a 25% chance of having another child with the same problem.

There is only a small chance that the child will develop systemic lupus erythematosus later in life.

3. Will getting pregnant cause a lupus flare?

Flares during pregnancy are most common during the first or second trimester or during the first few months following delivery.

Stress (both chronic and acute) plays a significant role in increasing your risk of a flare, so you must do all that you can to manage it.

The presence of lupus nephritis before conception also increases the chance of having complications during pregnancy. These concerns should be discussed with your doctor, as each person's situation and physical constitution is very different.

Most flares that do occur are mild and treated easily with corticosteroids.

The most common symptoms of these flares are:

-Arthritis

-Rash

-Fatigue

Remember, in some situations a flare is difficult to differentiate between lupus and pregnancy.

Certain abnormalities in lupus laboratory tests may be due to pregnancy rather than to lupus:

Approximately 33% of women with lupus will have a decrease in platelet count during pregnancyAbout 20% of women with lupus will have an increase in protein in the urine, or new occurrence of protein in the urine

These levels usually recover after delivery

4. When Is The Best Time To Get Pregnant?

The answer is simple: when you are at your healthiest.

Women in lupus remission have much less trouble than do women with active disease. It is important to remember that woman who conceive after 5 to 6 months of remission are less likely to experience a lupus flare than those who get pregnant while in a current flare.

Good health rules are essential:

-Eat well; you must follow a healthy diet for lupus (see site below for a specific diet for lupus guidelines)

-Take medications as prescribed

-Visit your doctor(s) regularly

-Don't smoke

-Don't drink

-Certainly don't use recreational drugs

-Listen to your body and follow its needs, (pushing through pain or fatigue is not advised, get the rest your body needs)

When you are in remission there is every reason to be hopeful that you can conceive a healthy child and maintain your health, especially if you are diligent with your health and get the support of medical (and alternative) health specialists.

Baby Blues Or Postnatal Depression


Postnatal depression (PND) commonly known as "baby blues" apparently can affect not only new mothers but their partners as well. We are talking about almost 16% of male population (USA statistics).

If women have this depression in the first few weeks after giving birth, men generally are very happy at this time until all the responsibilities, tiredness, fears and worries catch up with them and they become depressed in 3 to 6 months after their child is born.

This kind if depression in men is more likely to happen if their partner is also depressed. That's why doctors have to pay attention to the whole family when treating PND in female patients.

To get depression help is more problematic for new fathers. First, they might not realise they have PND, as it's an illness commonly associated with women. Secondly, they don't communicate with other new parents and share their experiences.

Remember PND can be treated in much the same way as an ordinary depression; the doctor might even get you antidepressants that are safe for breastfeeding mothers.

Here are a few tips if you suffer from PND:

1. Share your fears and anxieties with others, talk to your doctor as well, - this is common and you are NOT alone.

2. Do not feel guilty that you are a bad mother or father and don't be afraid that someone will take your baby away if you don't cope with the pressure well. This is NOT going to happen. People will help you to deal with your depression instead.

3. Try to sleep when and where you can, - you need your rest.

4. Don't try to do everything yourself, get help.

5. Eat healthy and wholesome food; you'll need all your energy for the baby.

6. Share responsibilities and fears with your partner.

7. Don't forget to spend time with your partner without the baby, hire a nanny once a week or ask your parents to look after your child for one evening.
